This per diem role allows you to serve Medicare Advantage plan members across Parke County, IN including areas like Rockville, Montezuma, and Marshall by providing essential in-home risk evaluations. Youll have the autonomy to choose your schedule and make a meaningful impact through comprehensive patient assessments without being responsible for treatment planning or medication prescribing.

Schedule:

• Flexible shifts available Monday through Sunday
• Create your own work schedule
• Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities:

• Perform annual home health risk assessments for Medicare Advantage enrollees
• Evaluate medical history, chronic conditions, and current medication regimens
• Deliver clear patient education on diagnoses and medications
• Identify care gaps and offer actionable recommendations
• Ensure documentation is complete and accurate after each assessment
• Conduct each visit within an average timeframe of 45 minutes
• Focus on patient-centered care and accurate reporting; no follow-up prescribing or plan development obligations

Qualifications:

• Active Nurse Practitioner License in Indiana
• Current CPR certification (BLS)
• National board certification (AANP or ANCC)
• MSN from an accredited nursing program
• At least 6 months of recent NP experience preferred
• Excellent communication and organizational abilities
