Play a vital role in supporting community health by performing essential in-home risk assessments for Medicare Advantage members. Schedule:

• Flexible shifts available seven days a week
• Self-scheduling for work-life balance
• Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Conduct comprehensive annual home health risk assessments in patients residences
• Assess and document medical history, current conditions, and medications
• Educate members on managing their health conditions and understanding medications
• Identify and report gaps in care, offering recommendations to address them
• Complete thorough evaluations without creating treatment plans or prescribing medications

Qualifications:

• Active Nurse Practitioner license in Indiana
• Current BLS (CPR) certification
• National board certification through AANP or ANCC
• Minimum of 6 months experience as a nurse practitioner preferred
• Master of Science in Nursing (MSN) from an accredited institution

This role offers competitive compensation of $80$90 per evaluation, along with a mileage stipend and compensation for administrative time. Youll visit Medicare Advantage members in their homes throughout Lafayette, West Lafayette, and surrounding locations in Tippecanoe County.
