Per Diem Home Health Nurse Practitioner (NP) - Brazil, Indiana

Embark on a rewarding role as a Nurse Practitioner specializing in home health risk assessments for Medicare Advantage plan members across Parke County, IN. This per diem opportunity provides the autonomy to create your own flexible schedule, with shifts available between Monday and Sunday and the option to commit to minimum weekly or bi-weekly hours. Assignments may be in a range of local areas, including Rockville, Montezuma, Marshall, and Adams Township.

Key Responsibilities:

• Conduct comprehensive annual or post-discharge home health risk assessments in patient residences
• Spend approximately 45 minutes on each evaluation, thoroughly reviewing medical history, current conditions, and medication usage
• Deliver clear education to patients regarding their health issues and prescribed medications
• Identify care gaps and make recommendations to ensure continuity and improvement of care
• Focus solely on assessmentsno requirement for treatment planning or prescribing medications

Qualifications:

• Active Nurse Practitioner license in Indiana
• MSN degree from an accredited school of nursing
• Current board certification through AANP or ANCC
• Current Basic Life Support (BLS/CPR) certification
• Six months or more of recent experience as a Nurse Practitioner is preferred

This position offers compensation of $80$90 per completed evaluation, plus a mileage stipend and pay for administrative time.
