Compensation:$53 - $56 hourly
Responsibilities:- Coordinate and manage comprehensive care plans for hospice patients, ensuring their comfort and dignity.
- Conduct regular assessments to monitor patient health, adjusting care plans as needed to meet evolving needs.
- Collaborate with interdisciplinary team members to deliver holistic, patient-centered care.
- Provide emotional support and guidance to patients and their families, fostering a compassionate environment.
- Educate patients and families about hospice care processes, empowering them with knowledge and confidence.
- Ensure timely and accurate documentation of patient care activities, maintaining compliance with healthcare regulations.
- Facilitate communication between patients, families, and healthcare providers to ensure seamless care transitions and continuity of care.
- Current Registered Nurse (RN) license in the state of New Jersey.
- Experience in hospice or palliative care, demonstrating a compassionate approach to patient management.
- Ability to develop and implement comprehensive care plans tailored to individual patient needs.
- Proven track record of collaborating effectively with interdisciplinary teams to enhance patient care.
- Strong communication skills to educate and support patients and families, fostering confidence and understanding.
- Proficiency in administering medications and treatments, prioritizing patient comfort and safety.
- Excellent documentation skills to maintain accurate and timely patient records, ensuring clear communication with the care team.
