Key Responsibilities\n

• Perform comprehensive client assessments to identify mental health needs and establish treatment goals in collaboration with the client.

• Facilitate ongoing communication between therapists, family members, guardians, and healthcare professionals to assess client progress and adjust care plans as necessary.

• Maintain real-time documentation of client interactions, focusing on observation and outcomes, to ensure compliance with healthcare regulations.

• Offer follow-up support through scheduled home visits, therapeutic sessions, and check-ins to foster a supportive recovery environment.

• Engage in interdisciplinary team meetings, offering insights from your perspective to refine treatment strategies.

• Develop and deliver training sessions to staff members on effective client engagement approaches and documentation practices.

• Advocate for clients by acting as a liaison within the mental health community, connecting clients with essential resources.

• Maintain accurate and confidential records structuring information for ready access by authorized stakeholders, leading to improved treatment outcomes.

Patient Population / Client Focus\n

In this role, you will serve a diverse patient population, including adults and youth with various mental health challenges such as anxiety, depression, trauma, and substance misuse. Qualifications\n

• A Master’s or Doctoral degree in Counseling, Psychology, or a related mental health field.

• Active and unrestricted Professional Counselor licensure (LPC) in the state of Texas.

• A minimum of 2 years of counseling experience, with preference given to clinical setting work.
