Key Responsibilities\n

• Conduct comprehensive psychosocial assessments through interviews, observations, and review of medical records to evaluate patients’ emotional, social, and environmental factors impacting their recovery outcomes.

• Provide empathetic counseling and emotional support to patients and their families dealing with adjustment issues, promoting mental health and resilience throughout the recovery process.

• Collaborate closely with the rehabilitation team to create and implement individualized treatment plans that respect the psychosocial needs of each patient.

• Facilitate patients' access to community resources, such as support groups, vocational rehabilitation services, and housing assistance, ensuring they have the tools necessary for a successful transition.

• Participate in discharge planning, supporting patients in making the smooth transition to home or other appropriate care settings, and ensuring continuity of care.

Patient Population / Client Focus\n

Your primary focus will be on individuals in our detoxification program, where you will work with patients facing various challenges, including substance use disorders, related mental health issues, and the societal impacts of addiction. Qualifications\n

• Master's degree in Social Work from an accredited program.
