This position is fully remote, providing you the flexibility to work from home while still being an integral part of our Detox Center team. You will engage closely with other healthcare professionals through telecommunication systems and regular online meetings. Key Responsibilities\n

• Conduct thorough psychosocial assessments to identify patients' emotional, social, and financial needs, enabling personalized treatment planning.

• Engage in both individual and group counseling sessions to address a variety of issues including adjustment, grief, and coping strategies.

• Work collaboratively with medical teams, including physicians and nurses, to integrate psychosocial care into patient treatment plans, ensuring holistic support.

• Coordinate with local community organizations to establish supportive post-discharge services that promote continuity of care for patients.

• Provide crisis intervention and emotional support to patients and their families, guiding them through challenging situations.

• Develop and implement effective discharge plans, connecting patients with appropriate external resources for their ongoing support.

• Maintain comprehensive and accurate documentation in accordance with organizational policies and legal requirements, ensuring confidentiality and compliance.

• Participate in departmental committees, contributing social work expertise to improve organizational practices and policies.

Patient Population / Client Focus\n

As a Licensed Clinical Social Worker at our Detox Center, you will work primarily with individuals struggling with substance use disorders. Qualifications\n

• A Master's degree in Social Work from an accredited institution.
