Responsibilities
- Plan and conduct segments of physical therapy treatment programs designed to restore and improve physical, social and mental functions, aligned with department objectives.
- Serve as co-leader in an interdisciplinary approach to patient groups, coordinating care with nursing, occupational therapy, speech therapy, and other team members.
- Perform patient evaluations, establish goals, implement individualized plans of care, monitor progress, and adjust as needed.
- Document patient status, progress notes, and care plans accurately; communicate updates to the care team and patient families as appropriate.
- Ensure patient safety and adherence to home health policies, infection control standards, and regulatory requirements.

Requirements
- Active Physical Therapist license in California
- Ability to work in a home health setting and participate in a travel assignment
- Strong communication and collaboration skills for interdisciplinary teams
- Eligibility to work in the United States as a traveler
- Demonstrated ability to engage patients in goal-directed therapy and maintain professional documentation
