In this role, you will provide high-quality therapeutic care to patients in the comfort of their homes, enabling them to recover from injuries, surgeries, or chronic conditions. Work Environment

This full-time position requires you to work onsite, primarily in a home care setting. You will interact with patients in their residences, applying your skills and clinical knowledge to assist in their recovery. Typically, your working hours will be from 7:30 AM to 3:30 PM or from 8:00 AM to 4:00 PM, Monday through Friday, ensuring you maintain a healthy work-life balance.

Key Responsibilities

Your duties will be comprehensive and patient-centered, including:

• Evaluating and treating patients recovering from injuries, surgeries, and chronic conditions within their homes.
• Creating individualized treatment plans tailored to each patient's specific needs and therapeutic goals.
• Employing a variety of therapeutic techniques including exercise regimens, manual therapy, and modalities to alleviate pain and increase mobility.
• Tracking patient progress meticulously, documenting treatment responses to ensure effective continuity of care.
• Revisiting and adjusting treatment plans based on ongoing assessments and patient feedback.
• Recommending and fitting patients with necessary assistive devices for mobility and daily living activities.
• Educating patients, caregivers, and family members on effective use of therapies and assistive modalities.
• Staying updated with the latest in physical therapy practices through ongoing professional development activities such as workshops and peer collaborations.

Patient Population / Client Focus

You will work with a diverse patient population, ranging from pediatric clients to older adults. This role involves treating various diagnoses, including but not limited to post-surgical rehabilitation, stroke recovery, orthopedic conditions, and chronic pain management. The aim is to empower patients to reach their maximum potential and regain their independence.

Qualifications

The ideal candidate will possess the following qualifications:

• A Doctorate in Physical Therapy (DPT) from an accredited institution.
• A valid and current state license to practice physical therapy.
• A minimum of two years of clinical experience in physical therapy, preferably in a home care setting.
• Exceptional communication skills, both written and verbal, to convey complex information effectively.
• Proficient computer and word processing skills for documentation and patient management.
• A flexible and dynamic approach to adapt to changing patient needs and environments.

Preferred Experience

While two years of relevant experience is required, we value attitude and aptitude. Prior experience in home health services or a community-based setting is a plus, although not mandatory. Our mentorship program is tailored to nurture new graduates eager to develop their skills further.
