Description

• Conduct thorough evaluations for patients in both inpatient and outpatient settings, crafting tailored treatment plans for each unique case
• Administer targeted physical therapy interventions focused on mobility, strength, balance, and overall function
• Collaborate closely with nurses, physicians, case managers, and a multidisciplinary team to enhance patient outcomes
• Guide patients and their families in understanding therapeutic goals, discharge planning, and home exercises
• Accurately document evaluations, treatments, and patients' progress in alignment with facility and regulatory requirements
• Engage in interdisciplinary care team discussions and rounds to ensure seamless, coordinated patient care

Education

• Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution

Certifications

• State licensure as a Physical Therapist
• CPR certification is highly preferred
• Additional specialized certifications (neurological, geriatric, etc.) are advantageous but not mandatory

Skills

• Previous experience is strongly favored
• Ability to treat patients with complex medical histories and mobility challenges
• Excellent communication and interpersonal skills
• Proficient in digital charting and clinical documentation practices
