Ascove Injury Prevention Rehab Center is seeking a dedicated and skilled Orthopedic Physical Therapist to join our team in Avenel, New Jersey. In this full-time role, you will be responsible for assessing, diagnosing, and creating tailored treatment plans for patients experiencing functional limitations due to orthopedic conditions. Work Environment

The physical therapy position is primarily onsite, focusing on home care delivery, enabling you to provide hands-on support for patients right in their living environments. You will work a standard schedule of 40 hours per week, which may include variable weekend duties to accommodate patient needs. Our team is composed of interdisciplinary professionals, allowing for rich collaboration and communication as we work together to help our patients thrive.

Key Responsibilities

• Conduct comprehensive evaluations of patients with orthopedic conditions to identify functional limitations and develop appropriate treatment plans.
• Implement individualized treatment programs, monitor progress, and adjust care as needed based on patient response and recovery.
• Provide expert consultation to healthcare providers regarding the continuum of care for shared patients.
• Educate patients and their families on therapeutic exercises, modifications, and lifestyle changes to maximize rehabilitation outcomes.
• Thoroughly document evaluations, treatment plans, and progress notes in alignment with state regulations and best practice standards.
• Collaborate with the interdisciplinary team members and maintain effective communication with physicians, support staff, and patients throughout the treatment process.
• Participate in community outreach initiatives, continuous quality improvement (CQI) efforts, and staff training programs to enhance service delivery and departmental capabilities.

Patient Population / Client Focus

This role focuses on a diverse patient population, primarily consisting of individuals recovering from orthopedic injuries, surgeries, and chronic conditions. Understanding and respecting the unique cultural backgrounds and needs of each patient will be essential in delivering effective care. Your approach will embody sensitivity and responsiveness to help each patient achieve their functional goals.

Qualifications

• A Bachelor's Degree in Physical Therapy is required; a Doctorate in Physical Therapy (DPT) is preferred.
• Current state licensure or eligibility for licensure as a Physical Therapist in New Jersey.
• Strong communication skills—both verbal and written—to effectively interact with patients, families, and colleagues.
• Proficiency in documentation and familiarity with electronic health record (EHR) systems.

Preferred Experience

• Previous experience in orthopedic rehabilitation settings is highly desired.
• Demonstrated ability to work collaboratively within a multi-disciplinary healthcare team.
• Experience with community outreach and health promotion initiatives is a plus.

Compensation and Benefits

The annual salary for this position ranges from USD 100,000 to 120,000, commensurate with experience and qualifications.
