Key Responsibilities\n

• Serve as a liaison connecting families and outpatient providers, facilitating cooperation and coordination of care.

• Interact with insurance companies to manage pre-certification and utilization management, ensuring clients receive necessary services.

• Participate in discharge and aftercare planning to promote continuous support for clients transitioning back into the community or school settings.

• Provide therapeutic meal support, assisting clients in modeling a healthy relationship with food and addressing any underlying issues related to eating behaviors.

• Deliver empathetic and affirming care tailored to each client’s unique circumstances, focusing on their intersecting identities and individual needs.

Patient Population / Client Focus\n

This role primarily focuses on school-aged children and adolescents who are facing various mental health challenges, including anxiety, depression, and behavioral issues. Qualifications\n

• A Master's degree in Social Work from an accredited institution.

• A valid LCSW license in the state of Georgia.
