About the Role Graham Healthcare Group, part of AHN View email address on click.appcast.io, is seeking a Registered Nurse to provide home health care during the day in Greenville, Pennsylvania. The role supports physicians, provides hands-on care and documentation, and serves as the primary point of contact for patients and their families. Responsibilities Deliver safe, accurate, patient-centered care in collaboration with interdisciplinary teams. Educate and empower patients and families to build confidence in care plans and support positive outcomes. Apply skills to solve challenges while ensuring thorough and compliant documentation. Act as the physician’s eyes and ears in the home, making critical decisions and providing high-quality, protocol-driven care. Maintain a structured, routine practice while remaining calm under pressure. Qualifications Active RN license in the state of employment. At least one year of clinical experience delivering quality home health care. Valid driver’s license and ability to travel to patient locations (approximately 20% of the day). Strong time-management skills, tech proficiency (EMR, mobile devices) and commitment to timely charting.
