Advanced Licensed Clinical Social Worker - Acute Care/Critical Care, Inpatient

Overview Advanced Licensed Clinical Social Worker - Acute Care/Critical Care, Inpatient (Full-time, 40 hours per week, Day shift). This role is within the inpatient acute/critical care setting requiring independent clinical judgment and a broad range of psycho-social interventions. Responsibilities Prepares and maintains electronic medical records in accordance with department and/or health system policies. Provides written and oral reports and hand-offs. Conducts psychosocial assessment of patients/families upon admission, annually, and/or as clinically indicated to evaluate social, emotional, and physical needs; identifies and addresses barriers to care. Collaborates with interdisciplinary health care teams to devise, develop, and implement patient/family psychosocial treatment plans; coordinates referrals to specialty and social services including entitlement programs and other care delivery systems; records progress notes and continuity of care. Helps patients/families cope with environmental and psychological issues of illness. Manages independently assigned caseload with varying clinical acuity. Engages in case finding for patients/families with potential psychosocial needs; attends team conferences and participates in interdisciplinary rounds as needed. Provides supportive counseling, grief counseling, crisis intervention, and other therapeutic services to assist patients and families facing challenges such as coping with illness and changes in prognosis or level of functioning. Provides psychoeducation to patients and families regarding adaptation to the patient\'s diagnosis, illness, treatment and/or prognosis; assists patients and families in the transition from youth to adult services. Acts as a department resource and may lead or contribute to department and hospital goals and development as assigned. Clinical supervision of unlicensed MSW, fellow, or student as applicable; may involve group, dyad or triad supervision per regulatory guidelines. Trains and on-boards new employees; supports pod members with procedural questions and complex cases. Represents the department or pod in hospital meetings to provide subject-matter expertise on workflow and quality improvement projects. Provides educational presentations in partnership with Clinical Educational Specialist to offer continuing educational opportunities for staff. Assigns cases to pod members; conducts department quality assurance tasks such as chart review and competency reviews. Minimum Qualifications Education: Master’s degree in Social Work from a CSWE-accredited program or equivalent (as demonstrated by licensure). Experience: Four (4) years of progressively responsible and directly related work experience. License/Certification: Valid license as a Licensed Clinical Social Worker (LCSW) issued by the California Board of Behavioral Sciences. Knowledge Ability to apply theories, principles, and practices of medical social work in a hospital or medical center setting. Ability to assess needs of patients and families using a data-based approach. Ability to manage a complex caseload while maintaining confidentiality and handling emergencies. Ability to provide written case narratives and reports; apply effective assessment, interviewing, and counseling techniques. Knowledge of medical center resources to access and provide for patient care needs; sensitivity to cultural considerations. Knowledge of child development and impact of injury and illness; knowledge of family dynamics and trauma-informed, family-centered approaches. Compensation Pay Range: Compensation is based on the level and requirements of the role. Salary within our ranges may be determined by education, experience, knowledge, skills, location, and abilities, as required by the role, and in alignment with market data. Typically, new team members join at the minimum to mid salary range. Minimum to Midpoint Range (Hourly): $57.69 to $64.68.
