LVN Nurse

The Licensed Vocational Nurse (LVN) provides skilled nursing care within a skilled nursing facility under the supervision of a Registered Nurse and Director of Nursing. This role includes assessing residents, administering medications following strict safety protocols, supervising CNAs and support staff, documenting care, and collaborating with the interdisciplinary team to ensure continuity and quality of care. The LVN acts as a resident advocate and educator, focusing on pain management and individual resident needs, with a minimum of two years of skilled nursing experience required. Responsibilities:

• Assess residents' conditions and nursing needs, including pain assessment and management.
• Administer medications following the eight medication rights to minimize errors.
• Document changes in resident condition and nursing care on progress notes and care plans.
• Perform waived point-of-care testing and interpret results according to policies.
• Communicate changes in resident conditions to physicians and follow up on physician orders promptly.
• Provide thorough shift reports and participate in walking rounds to ensure continuity of care.
• Implement individualized care plans developed by the interdisciplinary care team.
• Supervise CNAs and support staff, ensuring adherence to infection control and care standards.
• Educate residents and families on care plans, pain management, and discharge planning.
• Serve as a resident advocate, respecting psychosocial, cultural, ethnic, and spiritual needs.
• Assist in orientation and performance evaluations of new personnel as needed.
• Maintain calm and control in emergency or stressful situations.

• Graduate of an accredited LVN program.
• Current California Licensed Vocational Nurse (LVN) license.
• Current Basic Life Support (BLS) certification.
• Minimum of two years skilled nursing experience.
• Ability to read and communicate effectively in English.
• Basic computer skills; Electronic Health Record (EHR) experience preferred.

• Additional language proficiency.
• Experience with electronic health record systems.

• Must have skilled nursing experience.
• Role requires supervision of nursing assistants and support staff.
• Ability to work calmly and effectively in emergency or stressful situations.
• Focus on infection control procedures and resident advocacy.

• Part-time position, working two days per week: Wednesday night and Saturday night shifts.
