Key Responsibilities\n

• Act as a liaison between clients, their families, and outpatient providers to ensure seamless communication and transition of care.

• Interact with insurance companies to facilitate pre-certification and utilization management for visits and interventions.

• Lead discharge planning and aftercare follow-ups, contributing significantly to each client’s continuous health journey.

• Provide therapeutic meal support, exemplifying a healthy relationship with food while guiding clients in their recovery.

• Deliver empathetic and affirming care by considering clients' holistic needs, including their unique circumstances and identities.

Patient Population / Client Focus\n

This role focuses on clients undergoing detoxification, primarily those struggling with substance use disorders. Qualifications\n

Compensation and Benefits\n
