Key Responsibilities:

• Conduct annual home health risk assessments, averaging 45 minutes each, for Medicare Advantage members.
• Review and evaluate prior and current medical history, medical conditions, and medication records during home visits.
• Provide health education to patients regarding conditions and medication regimens.
• Identify gaps in care and offer evidence-based recommendations for improved health outcomes.
• Focus on assessment and patient educationno prescribing or treatment plan creation required.

Qualifications:

• Valid Nurse Practitioner license in VA
• Active CPR certification (BLS)
• Board certification via AANP or ANCC
• Minimum of six months experience as a Nurse Practitioner (preferred)
• Master of Science in Nursing (MSN) from an accredited program
