Job Description

Schedule/Hours:

• Flexible shifts available any day, Monday through Sunday
• Self-scheduling choose shifts that fit your lifestyle
• Minimum weekly or bi-weekly hourly commitment as required

Key Responsibilities:

• Perform home health risk assessments with each visit averaging 45 minutes
• Review and document patient medical histories and current conditions
• Assess medication history and usage with thorough documentation
• Educate patients on diagnoses, prevention, and medication management
• Detect care gaps and provide actionable recommendations
• No medication prescribing or treatment plan creation involved

Qualifications:

• Active Nurse Practitioner license in Virginia
• Current CPR (BLS) certification
• Board certified via AANP or ANCC
• Minimum of six months NP experience preferred
• MSN from an accredited nursing program
