Description

• Assess patients in inpatient or outpatient settings and craft personalized treatment plans tailored to their needs
• Implement physical therapy modalities aimed at enhancing mobility, strength, balance, and overall function
• Engage collaboratively with nursing staff, physicians, case managers, and rehabilitation experts to drive successful patient outcomes
• Provide education to patients and their families regarding therapeutic objectives, discharge plans, and necessary home exercises
• Document evaluations, treatments, and patient progress in accordance with facility policies and regulatory standards
• Participate actively in care team discussions and interdisciplinary meetings to ensure cohesive care delivery

Education

• Doctorate in Physical Therapy (DPT) or equivalent from an accredited institution

Certifications

• State licensure as a Physical Therapist
• CPR certification preferred
• Additional specialized certifications, such as in geriatrics or neurology, are beneficial but not mandatory

Skills

• Prior experience highly preferred
