Key Responsibilities\n

As an LPC at Rethink Mental Healthcare, your daily responsibilities will include:

• Conducting thorough ongoing assessments to identify and address the specific mental health needs of each patient.

• Maintaining open communication with supervisors, support staff, family members, and other relevant parties regarding client progress and treatment updates.

• Developing and providing a tentative schedule of appointments and home visits to ensure a structured approach to client care.

• Coordinating and performing home visits to facilitate in-home assessments, review client charts, interview staff, and analyze relevant data.

• Implementing and monitoring the effectiveness of personalized treatment plans, adjusting as necessary based on client feedback and progress.

• Participating in interdisciplinary team meetings to collaborate on client care strategies and share insights with fellow healthcare providers.

• Documenting monthly and quarterly progress notes to track client development and inform stakeholders of results.
\n
• Training and certifying team members in the use of program plans and documentation practices to ensure high-quality care delivery.

• Providing ongoing follow-up support through interviews and home visits to address any changes in a client’s needs or circumstances.

• Maintaining accurate and organized records that are readily accessible to authorized parties and stakeholders.
