Work Environment\n

This position is entirely remote, enabling you to operate in a flexible environment while engaging closely with our hospice patients through comprehensive telehealth platforms. You will interact with a diverse patient population, primarily those experiencing mental health conditions related to chronic illnesses, grief, and emotional distress. Regular team meetings will be conducted via video conferencing to foster communication and collaboration amongst team members.

Key Responsibilities\n

• Provide high-quality psychotherapy and mental health services tailored to patients struggling with conditions such as anxiety, depression, post-traumatic stress disorder (PTSD), and adjustment disorders related to terminal illness.

• Conduct thorough assessments of patients' mental health status, evaluating their emotional, behavioral, and social functioning to build effective treatment plans.

• Develop personalized treatment plans that are evidence-based and culturally sensitive, ensuring they meet the unique needs of each patient.

• Maintain ongoing communication with healthcare professionals, including psychiatrists and primary care providers, to coordinate a comprehensive approach to care.

• Regularly monitor patients’ progress, adjusting treatment plans based on patient feedback and clinical observations to optimize outcomes.

• Provide education and resources to patients and their families regarding mental health conditions, available treatments, and community support services.

• Uphold high ethical standards in practice by maintaining patient confidentiality and obtaining informed consent for therapeutic interventions.

Patient Population / Client Focus\n

The primary population for this role includes patients in a hospice setting, many of whom are experiencing a range of mental health challenges stemming from chronic health conditions, terminal diagnoses, or the process of grief and loss. Your role will be integral in addressing their psychological needs while providing compassionate support to their families, helping to create an environment of understanding and dignity during difficult life transitions.

Qualifications\n

• Master's degree in Social Work from an accredited institution, with current licensure as a Licensed Clinical Social Worker (LCSW) in Arizona.

• A minimum of 2 years of experience providing psychotherapy within a clinical setting, preferably with vulnerable populations facing mental health challenges.

• Demonstrated clinical expertise in developing treatment plans and conducting patient assessments.

• Exceptional communication and interpersonal skills, enabling effective relationship building with patients and collaboration with multidisciplinary teams.

• Familiarity with electronic health records (EHRs) and additional healthcare management software to effectively document patient interactions and coordinate care.

• Knowledge of mental health treatments and an understanding of the complex needs of hospice patients.

Compensation and Benefits\n
