Key Responsibilities\n

• Conduct comprehensive bio-psychosocial evaluations to assess patients’ emotional, social, and environmental strength and barriers to treatment, tailoring interventions accordingly.

• Develop and implement individualized treatment plans utilizing various evidence-based modalities, including both individual and group therapy sessions.

• Monitor and evaluate patient progress, adjusting treatment strategies as necessary to optimize health outcomes and foster engagement in their care.

• Document social work activity meticulously in electronic medical records in compliance with departmental standards and regulatory requirements.

• Collaborate with medical and support staff to create coordinated care plans that respect the patient’s cultural, social, and individual needs.

• Deliver education and resources to empower patients and their families in understanding and adhering to health care recommendations.

• Advocate for patients' rights and needs within the community, ensuring access to essential services and supports.

• Maintain an awareness of community resources and refer patients to appropriate services that can enhance their psychosocial well-being.

• Engage in continuous professional development activities and stay updated on best practices in clinical social work.

Patient Population / Client Focus\n

This position primarily involves working with patients within a hospice setting, offering psychological support to adults facing terminal illness, as well as providing support to their families. You will need to show sensitivity to patients' grief and varying coping mechanisms, addressing life transitions unique to end-of-life situations.

Qualifications\n

To be successful in this role, you must possess:

• A Master’s Degree in Social Work from an accredited institution alongside a current license from the State Board of Social Work Licensure, specifically as an LCSW.

• Demonstrated understanding of psychological principles and frameworks, especially as they pertain to aging processes and human development stages.

• Proficiency in assessing and addressing the diverse needs of patients, considering cultural contexts and social dynamics.

Preferred Experience\n

While not essential, the following experience is highly preferred for candidates:

• A minimum of 1+ years of clinical experience in behavioral health settings, with direct experience working with patients of all ages, particularly in hospice or palliative care.

Compensation and Benefits\n

We offer a competitive salary range of USD 85,000 – 100,000 annually, along with a SIGN-ON BONUS to help you get started.
