Must-Have: Experience in medical/surgical nursing and/or home health care.

You will assess a patient's health status and complete nursing interventions in collaboration with the interdisciplinary team.
Under the direction of an RN Case Manager, you will be responsible for direct nursing care to hospice patients.
Educate patients and families in accordance with the care plan established by the interdisciplinary team.
Make home visits to assist with the provision of care to meet patient and family needs.
Communicate changes in patient condition and needed changes to the plan of care to appropriate team members and caregivers.
Assist the RN or medical provider in performing specialized duties related to end-of-life care.
Observe, document, and report to the RN as appropriate the patient's physical and emotional needs and reactions to care.
