SPECIAL SERVICE FOR GROUPS INC. seeks an Intensive Case Manager for Permanent Supportive Housing in Los Angeles. Under the Program Manager, you will provide participant-centered, housing-focused case management, coordinating services across health, behavioral health, and social supports to move participants toward permanent housing retention. Your role includes comprehensive assessments, care planning, and ongoing coordination with housing providers, healthcare teams, and county partners, with a #J-18808-Ljbffr Special Service for Groups, Inc.
