Key Responsibilities\n

• Conduct comprehensive psychosocial assessments to identify patients' emotional, social, and financial needs, working closely with medical teams for integrated care.

• Provide both individual and group counseling sessions tailored to address various issues including adjustment disorders, grief, and coping strategies for trauma.

• Develop discharge plans that connect patients with appropriate community resources, ensuring smooth transitions from in-care to out-of-care environments.

• Facilitate crisis intervention and emotional support during critical moments, coordinating efforts with interdisciplinary teams to optimize patient outcomes.

• Document patient interactions and treatment plans accurately in accordance with regulatory and institutional standards.

• Engage in ongoing communication with community organizations to arrange post-discharge support, promoting continuity of care.

In this role, you will serve a diverse patient population experiencing anxiety, trauma, and related mental health challenges. Qualifications\n

• A Master’s degree in Social Work from an accredited program is mandatory.

Compensation and Benefits\n
