Position Summary

This position involves assessing, planning, and implementing nursing care for residents in a skilled nursing environment under the supervision of a Registered Nurse and the Director of Nursing. The role requires administering medications while adhering to the eight medication rights to minimize the risk of medication errors and ensuring continuity of care between shifts.

Key Responsibilities

• Assess the resident's condition and identify nursing needs; prescribe appropriate nursing actions.
• Effectively assess and reassess pain, utilizing suitable pain management techniques.
• Educate residents and their families on pain management practices.
• Document changes in residents' conditions and nursing care in progress notes and care plans.
• Perform waived (point-of-care) testing according to established policies and procedures, interpreting results, and taking necessary actions.
• Administer medications accurately while adhering to established medication rights to reduce potential errors.
• Meet current documentation standards and policies for nursing care.
• Notify physicians of any changes in resident conditions and ensure appropriate follow-up actions are taken.
• Conduct and document physician orders in a timely manner.
• Provide thorough reports to oncoming shifts through participation in walking rounds and accurate documentation.
• Implement care plans based on assessments and interdisciplinary team goals.
• Maintain respectful and dignified interactions with residents and their families, addressing their psychosocial, cultural, ethnic, and religious/spiritual needs.
• Supervise and guide other healthcare team members to achieve care plans effectively.
• Demonstrate technical competence and manual skills in line with the LVN scope of practice.
• Act as a resident advocate, safeguarding residents' rights.
• Exhibit creative problem-solving and innovative thinking.
• Engage with residents, families, co-workers, managers, and other departments in a friendly and knowledgeable manner.
• Involve residents and families in the planning of care, treatment, teaching needs, and discharge strategies.
• Function as a leader for CNAs, delegating responsibilities to enhance the nursing unit's performance.
• Participate in the orientation of new personnel and assist in performance evaluations as needed.
• Follow infection control protocols and supervise supportive staff in adhering to these standards.
• Maintain composure in emergency or stressful situations, fostering confidence among staff and residents.
• Adhere to all facility policies and procedures consistently.
• Demonstrate genuine interest and appreciation for working with seniors.
• Organize workload independently to ensure all required functions are completed efficiently.

Qualifications

• Graduate of an accredited LVN program.
• Current Licensed Vocational Nurse in California.
• Current Basic Life Support (BLS) certification.
• Ability to read and communicate effectively in English; additional languages are preferred.
• Basic computer proficiency; experience with Electronic Health Records is preferred.

Work Schedule

Night Shift, part-time, Wednesday and Saturday nights.
