Key Responsibilities\n

• Conduct age- and culturally-appropriate biopsychosocial assessments that evaluate mental health, substance abuse issues, and related medical needs for patients in a primary care setting.

• Provide behavioral health services including individual counseling, couples therapy, family support, comprehensive intake assessments, and crisis intervention strategies.

• Document clinical interactions and patient progress through electronic means, ensuring all notes regarding evaluations, treatment plans, and interventions meet regulatory standards.

• Collaborate closely with interdisciplinary teams to facilitate referrals to community resources, social support services, and local medication assistance programs.

• Engage in quality improvement initiatives by collecting data pertinent to patient outcomes and treatment efficacy, helping to shape best practices within the organization.

• Respond to escalated patient needs with timely interventions, coordinating care as necessary to ensure optimal client well-being.

Qualifications\n
