Key Responsibilities\n

• Facilitate coordination with community organizations to ensure seamless transition post-discharge and ongoing support services, helping patients navigate their care journey.

• Offer emotional support and education to patients and families about treatment options, resources, and the goals of hospice care.

• Engage in crisis intervention, providing timely support to patients and families facing urgent emotional challenges.

• Document all patient interactions and treatment plans accurately, meeting compliance standards set forth by hospital policies and legal requirements.

• Collaborate with interdisciplinary teams to optimize patient outcomes while participating in hospital committees that align with social work initiatives.

Qualifications\n
