Key Responsibilities\n

• Educate patients and families regarding community resources and the support systems available post-rehabilitation to enhance their quality of life.

• Take an active role in discharge planning, ensuring smooth transitions for patients moving back home or into alternative care settings.

The salary for this position ranges from USD 75,000 to 100,000 annually, commensurate with experience and qualifications.
