Key Responsibilities\n

• Conduct comprehensive intake assessments to evaluate client needs and mental health status.

• Develop, implement, and modify individualized treatment plans that accurately reflect clients' personal goals and therapeutic needs.

• Provide direct counseling services via telehealth, addressing issues such as depression, anxiety, grief, and trauma.

• Establish trusting, therapeutic relationships with clients, fostering an environment of safety and support.

• Collaborate with interdisciplinary teams, including healthcare providers and support staff, to ensure a holistic approach to client care.

• Offer crisis intervention and immediate support to clients in emotional distress, connecting them to appropriate resources as needed.

• Document client progress through detailed case notes and treatment summaries, adhering to ethical and legal standards of practice.

Patient Population / Client Focus\n

In this role, you will work with a diverse range of clients, primarily individuals and families receiving hospice care. This population often deals with complex emotional responses due to terminal illness, including grief, fear, and adjustment issues. Your expertise will guide clients and their families through these challenging times, helping them to navigate their emotions and find peace and fulfillment in their remaining time together.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

Compensation and Benefits\n

We offer a competitive salary ranging from $70,000 to $90,000 per year, depending on experience and qualifications.
