Work Environment\n

This position is entirely remote, offering flexibility and the ability to work from any location. However, the role will primarily focus on the services we provide at our Hospice Center. Our team delivers a holistic approach to care, engaging in collaborative practices that integrate social work with medical and behavioral health services, ensuring that families receive the support they need in their own environments.

Key Responsibilities\n

• Conduct comprehensive evaluations and assessments of children enrolled in early intervention programs to identify their developmental needs.

• Collaborate in the development and implementation of Individualized Family Service Plans (IFSP), ensuring they are tailored to a child's unique developmental goals.

• Monitor and evaluate outcomes, adjusting service strategies as necessary while actively participating as a team member.
\n
• Provide direct support and counseling to families, aiding them in enhancing their child’s development through evidence-based practices.
\n
• Ensure compliance with all relevant Federal and State regulations, maintaining the highest standards of care and client confidentiality.

• Foster positive relationships with children and families to create a supportive and effective learning environment.
\n
• Utilize a coaching model and practices that promote development within natural learning environments.
\n
• Offer families strategies and resources designed to support their child's developmental milestones.

Patient Population / Client Focus\n

This position primarily involves working with infants and toddlers—children from birth to three years old—who have disabilities or developmental delays. You will engage with their families, empowering them to become active participants in their child’s developmental journey. Our goal is to create a nurturing, enriching environment that fosters improvement in physical, cognitive, social/emotional, and communication skills.

Qualifications\n

• A Master's Degree in Social Work or a Doctorate in Social Work is required.

• Current licensure as a LCSW by the Department of Public Health.

• A valid driver's license is necessary to facilitate travel when required.
\n
• Experience and confidence in working with diverse families and adapting to their individual learning styles.

• An understanding of and commitment to Natural Learning Environment Practices.

Preferred Experience\n

Compensation and Benefits\n
