Clinical Case Manager/ ASL Therapist

Clinical Case Manager/ ASL Therapist DCCS Program Adult Division Agency Overview Felton Institute responds to the needs of our community by providing innovative, evidence-informed social services that transform lives. Program Description Deaf Community Counseling Services (DCCS) was formerly known UCSF Center on Deafness (UCCD) and is in three offices: San Francisco, Alameda, and Berkeley. Ten years ago, DCCS partnered with Felton Institute (formerly Family Service Agency of San Francisco- FSA), which is the oldest nonprofit agency in San Francisco County. For 103 years, Felton Institute responds to human needs with cutting‑edge social services and mental health programs that combine the best social science research with cultural sensitivity, a deep respect for the individuals served, and a commitment to social justice. DCCS is of, by, and for Deaf, Hard of Hearing, Deaf Blind, Deaf Plus (Deaf with other disabilities), and Late Deafened by empowering Deaf & Hard of Hearing clients to live self‑sufficient lives through a transformative focus on their overall well‑being. DCCS supports and educates the community on the importance of healing, integrating care for their cognitive, emotional, and behavioral health. DCCS provides clinical assessment, targeted case management, crisis intervention, psychotherapy, rehabilitation counseling, psychiatric services/medication support, and DCCS has been networking with primary care providers and schools in surrounding counties committed to quality and innovation for providing comprehensive care within the Deaf and Hard of Hearing community. Job Description Clinical Case Manager/Therapist is under supervision of the clinical Supervisor and is responsible for providing direct services to Deaf, Hard of Hearing, Deaf-Blind, and Late-Deafened clients. Clinical Case Manager/ ASL Therapist is also expected to report to Program Manager for program related duties. The Therapist must have licensure (LCSW, MFT, LPCC, or on track to full clinical licensure). Required to have licensure before joining the DCCS program. Specific Responsibilities Assessing and developing plan of care for clients to assigned caseload Provide therapy, rehabilitation counseling (skill building mental health), targeted case management (advocating, help clients secure benefits, housing, day treatment program, substance use treatment, and attending IEPs on behalf of students), and crisis intervention Document progress notes related sessions with clients Attend weekly staff meeting Attend training for your professional growth Attend outreach events on occasion, virtually or in person when able. Provide relevant presentations or workshops to the community Works closely with DCCS clinical case managers, case managers, psychiatric nurse practitioners and/or psychiatrist Community Outreach and engagement with other service providers Provides information regarding community resources to the general public as needed. Minimum Requirements and Qualifications Fluency in ASL (American Sign Language) and knowledge and familiarity of Deaf, Deaf[1]Blind, Hard of Hearing and Late Deafened Cultures. Master's degree or higher in Social Work, Counseling, or Psychology, LCSW, MFT, LPCC or on track preferred. Comprehensive range of mental health and substance abuse counseling, assessment, and crisis intervention skills. Ability to work independently and remotely when needed, and cooperatively on a multidisciplinary team, and to work with clients of various ethnic backgrounds. Knowledge of DSM 5 Valid CA Driver's License and reliable transportation Valid Motor Vehicle Insurance Ability to manage and maintain strong structure and boundaries when transporting clients in a motor vehicle. Additional Information Reports to: Program Manager & Clinical Supervisor Job Type: Full-Time Work Schedule: Onsite FLSA Status: Exempt Salary Range: $85,000 - $90000 Annually (DOE) Benefits (subject to election and eligibility): Fully-Paid Medical and Dental Plans; Health Reimbursement Card, Paid Time Off (personal, vacation, and sick time), 403B with company match.
