Work Environment

This position is based onsite within our patients' homes, allowing for personalized, one-on-one therapeutic sessions that cater directly to individual needs. You'll engage in a collaborative approach, alongside a multi-disciplinary team, to maximize patient outcomes and redefine recovery experiences.

Key Responsibilities

Your responsibilities as a Physical Therapist will include:

• Conducting comprehensive evaluations of patients referred by physicians to establish personalized treatment plans.
• Utilizing evidence-based practices to develop and execute therapeutic interventions that address each patient's unique condition and goals.
• Providing skilled therapeutic care directly in the patient's home, ensuring the treatment aligns with the physician's plan of care.
• Engaging in ongoing assessments to track patient progress and adjust treatment plans as necessary.
• Consulting with and educating patients and their caregivers on exercises, mobility techniques, and lifestyle modifications to support ongoing recovery.
• Maintaining effective communication with a broader healthcare team to coordinate patient care and optimize treatment outcomes.
• Meeting or exceeding minimum utilization goals and contributing to the organization's overall effectiveness and mission.

Patient Population / Client Focus

As a Physical Therapist with Dynamic Strength, you will work with a wide range of patient populations, including orthopedic, pediatric, neurologic, and sports medicine cases. Qualifications

To qualify for this position, candidates must possess:

• A Bachelor's Degree in Physical Therapy; a Doctorate in Physical Therapy (DPT) is preferred.
• A valid state license in Physical Therapy to practice in Virginia.
• Basic Life Support (BLS) Certification from the American Heart Association.
• Strong written and verbal communication skills for effectively interacting with patients and their families.
• Proficiency in word processing and basic computer skills.
• A demonstrated desire to positively engage with patients and the community.
• Ability to build trust-based relationships that facilitate effective rehabilitation.

Preferred Experience

While previous experience in home health care is advantageous, it is not mandatory. However, experience in a clinical setting dealing with diverse patient populations will be beneficial. You should also have a thorough understanding of various therapeutic modalities and evidence-based treatment approaches.
