Work Environment

As part of our home care team, you will operate primarily in patients' homes, allowing you to establish strong one-on-one relationships in a comfortable and familiar setting. Key Responsibilities

In this role, you will:

• Adhere to state practice acts and regulations governing physical therapy practices, ensuring compliance in all aspects of care.
• Conduct comprehensive examinations and evaluations, developing tailored treatment plans for patients experiencing functional limitations and disabilities.
• Implement evidence-based treatment protocols based on initial evaluations, patient feedback, and ongoing assessments of their condition.
• Effectively collaborate with other healthcare providers as needed, recognizing when to adjust treatment strategies or make referrals for additional medical care.
• Document patient interactions meticulously, providing clear and timely records of evaluations, treatment plans, and progress updates to maintain continuity of care.
• Complete discharge planning and follow-up assessments, ensuring a smooth transition for patients as they move beyond physical therapy.
• Stay up to date with corporate and clinical compliance training to meet all local, state, and federal regulations.
• Participate in team meetings and contribute to initiatives designed to improve patient access and quality of care.

Patient Population / Client Focus

Our patient population primarily consists of individuals with orthopedic conditions, chronic pain syndromes, and post-surgical recovery needs. By focusing on personalized treatment plans and therapeutic exercises, you will determine the most effective techniques to help our patients regain their independence and improve their quality of life. You will be a key figure in supporting patients through rehabilitation, ensuring their treatment is aligned with their goals and recovery timelines.

Qualifications

To be considered for this role, candidates must possess the following:

• A Doctorate of Physical Therapy (DPT) from an accredited institution.
• A valid state license to practice as a Physical Therapist, as required by the Physical Therapy Examining Committee.
• A minimum of two (2) years of outpatient orthopedic experience.
• Current CPR certification to ensure emergency preparedness.

Preferred Experience

While the minimum qualifications are vital, candidates with experience in geriatrics, post-operative care, and community-based rehabilitation programs will be given preference. Familiarity with electronic health record systems and telehealth practices will also be considered advantageous in our evolving healthcare landscape.
