Work Environment

This position is primarily onsite, providing care directly within patients' homes. Working in a home care environment allows you the flexibility to engage with patients on a personal level and to observe their living conditions, which can be critical in tailoring rehabilitation plans effectively. Key Responsibilities

• Conduct comprehensive evaluations of patients' physical therapy needs, developing personalized treatment plans in collaboration with their medical team.
• Implement and adjust treatment plans using research-driven methodologies to meet patient goals, while ensuring alignment with physicians' directives.
• Utilize various therapeutic modalities, including exercises, manual therapy, and neuromuscular techniques, to help patients regain function and alleviate pain.
• Provide education and support to patients and their families to ensure understanding of therapeutic processes, goals, and home exercise programs.
• Demonstrate exceptional customer service throughout the rehabilitation journey, building trust and rapport with patients and their caregivers.
• Maintain accurate and timely documentation of patient progress and treatment interventions in accordance with state regulations and facility policies.
• Work collaboratively with other healthcare professionals to coordinate comprehensive care and maximize patient outcomes.

Patient Population / Client Focus

As a Physical Therapist at Advent Care, you will work with a diverse range of patients across various specialties, including orthopedic, pediatric, neurologic, and sports medicine populations. This role requires a genuine interest in helping individuals navigate the recovery process, ensuring they regain their functional abilities and improve their quality of life.

Qualifications

• Bachelor's degree in Physical Therapy from an APTA or CAPTE-approved program.
• Current state license to practice as a Physical Therapist.
• Basic Life Support (BLS) certification from an accredited organization, such as the American Heart Association.
• Excellent verbal and written communication skills for effective patient interaction and teamwork.
• Proficiency in documentation and treatment planning using electronic health records (EHR) systems.

Preferred Experience

• Two or more years of experience in a clinical setting, particularly in home health or outpatient care.
• Doctorate of Physical Therapy (DPT) preferred, with current state licensure as a DPT.
• Experience in patient education and caregiver training to promote understanding and adherence to therapeutic regimens.
• A track record of establishing strong professional relationships with patients and their families.
