Key Responsibilities\n

• Collaborate with the healthcare team to develop individualized treatment plans, including discharge strategies that are based on thorough psychosocial assessments and accepted social work practices.

• Conduct comprehensive assessments to identify patients’ needs and implement appropriate interventions, modifying strategies as necessary based on ongoing evaluations.

• Maintain accurate and timely documentation of all clinical activities, including progress notes and treatment reviews, ensuring compliance with departmental policies and legal requirements.

• Provide crisis intervention services, employing strategies to assess and manage patients facing emotional and behavioral challenges, while ensuring their immediate safety and well-being.

Qualifications\n

Compensation and Benefits\n
