Key Responsibilities\n

• Conduct comprehensive psychosocial assessments and collaborate with an interdisciplinary team to develop holistic care plans tailored to patients’ needs.

• Coordinate with community organizations to facilitate post-discharge services and ensure continuity of care for patients leaving the hospice setting.

• Offer emotional support and educational resources to patients and families regarding treatment options and available healthcare resources.

• Participate actively in hospital committees, lending your expertise on social work-related issues to influence best practices.

• Deliver crisis intervention and counseling, ensuring that patients and families are supported during challenging times.

• Develop and implement effective discharge plans and connect patients with appropriate community resources.

• Maintain thorough and timely documentation to ensure compliance with hospital policies and legal requirements.

Patient Population / Client Focus\n

In this role, you will work primarily with patients facing terminal illness and their families, providing emotional and psychological support during one of the most challenging times in their lives. Qualifications\n

• Master's degree in Social Work from an accredited institution.

• Active state licensure as a Licensed Independent Clinical Social Worker (LICSW).

Compensation and Benefits\n

The compensation for the Licensed Independent Clinical Social Worker position ranges from USD 70,000 to 100,000 annually, commensurate with experience.
