Key Responsibilities\n

• Conduct thorough psychosocial assessments of patients and families to understand their emotional, social, and environmental strengths and challenges, particularly in relation to serious illness or end-of-life considerations.

• Develop and implement individualized treatment plans that incorporate therapeutic interventions tailored to the unique needs of each patient.

• Collaborate closely with healthcare teams, engaging both patients and their families in care planning and intervention strategies to support effective patient transitions.

• Facilitate discharge planning by partnering with multidisciplinary teams, ensuring that all necessary resources are arranged for a smooth transition out of hospice care.

• Provide education, counseling, and support to patients and families regarding coping strategies for illness and adjustments to life-changing situations.

• Utilize various therapeutic modalities, including crisis intervention, brief and long-term therapies for individuals, groups, and families.

• Act as a clinical consultant to fellow staff members and contribute educational sessions to enhance team knowledge on psychosocial care frameworks.

• Assess oncology patients for mental health concerns, such as anxiety and depression, and provide short-term counseling as needed.
\n
• Participate in on-call rotations to address after-hour emergencies and facilitate necessary interventions.

• If needed, facilitate involuntary examinations under relevant legislation, ensuring patient safety and compliance with legal requirements.

• Engage in the community by providing educational resources, volunteer opportunities, and outreach services aimed at enhancing support systems for cancer survivors and their families.

The primary client population for this role includes patients receiving hospice care, particularly those confronting terminal illnesses, along with their families. Qualifications\n

• Must hold a valid Licensure as an Independent Clinical Social Worker (LICSW) in Massachusetts.

Compensation and Benefits\n

The annual salary for this role ranges from USD 70,000 to 100,000, commensurate with experience and qualifications.
