Community Outreach Specialist
CAN Community Health
CAN Community Health in Sarasota, FL is hiring a Mobile HCV Community Health Worker I to deliver HIV, STI, and Hepatitis C testing, education, and linkage to care on mobile and clinic units. You will perform risk-reduction counseling, support clinical visits, and help patients navigate care, while documenting all activities in compliance with HIPAA and CAN policies.
