RN: Case Manager

Job Description This 13-week contract positions you as an inpatient RN Case Manager at a major hospital in Fort Wayne, Indiana. You will be responsible for collaborating with the care team to conduct comprehensive patient assessments, drive stay progression, and facilitate discharge planning to ensure smooth patient transitions. The role requires expertise in hospital utilization management, with proficiency in Epic, InterQual, and MCG. Shifts are 8 hours daily, Monday through Friday, with participation in weekend rotations. Local candidates with Indiana or compact licenses are preferred to take advantage of reduced rates, and recent experience of 2+ years in similar roles is essential. You’ll be working primarily in one unit but may float to others as needed, with a focus on efficient, compliant documentation and patient flow management. The work environment demands independent and collaborative skills, and a certification in Case Management is highly preferred.
