Key Responsibilities\n

• Conduct thorough assessments of clients' mental health needs and develop individualized treatment plans based on their unique circumstances.

• Maintain consistent follow-up through home visits, telehealth sessions, and regular progress notes to track the client’s development and adjust treatment plans as necessary.

• Participate actively in Interdisciplinary Team Meetings to discuss client progress and collaborate on strategies for improvement.

• Maintain accurate and comprehensive documentation in compliance with legal, ethical, and organizational standards.

• Organize and lead training sessions for other team members, ensuring they are well-equipped to adhere to the program’s goals and documentation requirements.
