Key Responsibilities\n

• Conduct comprehensive assessments of clients' social, emotional, and environmental challenges to inform individualized service planning.

• Design and implement personalized treatment plans that address client-specific goals, facilitating their access to relevant community resources.

• Deliver various therapeutic techniques to support individuals and families experiencing anxiety, depression, grief, and other mental health challenges.

• Provide crisis intervention services, including risk assessments and safety planning when necessary, ensuring immediate support for clients in distress.

• Collaborate with local community organizations and service agencies to coordinate care for clients, advocating for their needs and rights.

• Engage in community outreach initiatives to raise awareness about available services and promote mental health education.

Patient Population / Client Focus\n

Your primary focus will be on individuals and families facing significant life stressors, including those related to mental health, chronic illness, and end-of-life care. Working within our hospice center, you will provide emotional and psychological support to clients during some of their most challenging times. Your role is critical in helping them navigate their circumstances, facilitating access to resources that enhance their quality of life.

Qualifications\n

• A Master’s degree in Social Work (MSW) from an accredited institution is required.

• An active state licensure as a Licensed Clinical Social Worker (LCSW) is mandatory.

Compensation and Benefits\n
