Key Responsibilities\n

• Conduct comprehensive psychosocial assessments through structured interviews, objective observations, and thorough evaluations of medical records. This analysis will help identify emotional, social, and environmental factors critically influencing each patient's recovery journey.

• Offer individual and family counseling sessions aimed at addressing the emotional ramifications of addiction and the challenges of adjustment to sobriety.

• Participate actively in multidisciplinary team meetings to collaborate with healthcare professionals, designing personalized treatment plans that incorporate psychosocial considerations.

• Guide patients in accessing essential community resources, such as support groups, vocational rehabilitation services, and housing assistance, ensuring they have a solid network for ongoing support.

• Educate patients and families about post-rehabilitation services that can aid in avoiding relapse and support sustained recovery.

• Engage in discharge planning, ensuring seamless transitions from the Detox Center to either home care or alternative treatment facilities.
