Provide counseling and emotional support to patients and caregivers. Assist families in accessing community resources, financial aid, and legal support. Collaborate with the interdisciplinary hospice team to ensure holistic care. Maintain accurate and timely documentation in compliance with regulations. Requirements: Certified Social Worker - Required 2+ years of MSW experience in Home Health or Hospice setting - Required Experience in Hospice or palliative care - Required Strong interpersonal and counseling skills. Valid Drive License and reliable transportation.
