Crisis Case Manager, Social Services

Care New England's Providence Center CSU team is seeking a dedicated Case Manager to conduct screenings, assessments, counseling, and referrals for clients. The role emphasizes crisis intervention and coordination with in-house and external services. You will schedule appointments, assist with benefits applications, and complete required documentation while maintaining patient confidentiality and following organizational protocols.
