Key Responsibilities\n

• Conduct thorough assessments to identify the counseling needs of clients, particularly in the areas of mental health and co-occurring substance use disorders.

• Utilize DSM (Diagnostic and Statistical Manual) criteria to accurately diagnose substance abuse issues and determine the appropriate levels of care based on ASAM (American Society of Addiction Medicine) standards.

• Design and implement personalized treatment plans, focusing on substance abuse counseling tailored to the unique circumstances of each client.

• Develop comprehensive discharge and continuity of care plans to facilitate a smooth transition for clients to the necessary levels of ongoing support.

• Maintain meticulous documentation of all counseling interactions, sessions, assessments, and follow-ups in clients' clinical records in compliance with regulatory standards.

• Manage a caseload of clients, ensuring effective case management and regular updates for utilization reviews.

• Coordinate support services for clients and their families, keeping detailed records of provided services in clinical documentation.

• Establish and maintain strong communication channels with referral sources, including educators, legal representatives, and community organizations.

• Implement interventions for crisis-oriented issues, effectively utilizing a multi-disciplinary team approach for cases requiring additional support.
