Key Responsibilities\n

• Act as a liaison between patients, families, and outpatient providers, ensuring transparent communication and comprehensive care.

• Interact with insurance companies to facilitate pre-certification and manage utilization effectively, advocating for patient needs.

• Participate in discharge and aftercare planning to help clients transition successfully from our Hospice Center to home-based care.
\n
• Provide therapeutic meal support to clients, helping them develop a healthy relationship with food while modeling positive behaviors.
\n
• Deliver empathetic and affirming care tailored to the unique circumstances of each client, considering their intersecting identities.
\n
• Maintain accurate documentation of patient interactions, treatment plans, and progress notes in accordance with clinic policies and state regulations.

Patient Population / Client Focus\n

In this role, you will primarily work with older adults who are facing end-of-life challenges, along with their families and caregivers. This population often experiences complex emotional and psychological issues, requiring sensitivity, understanding, and specialized care. Your role will help ensure these clients receive compassionate support during difficult transitions, thereby contributing to their overall quality of life.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution is required.

• Possession of a valid LCSW license in the state of California is mandatory.

Compensation and Benefits\n
