Key Responsibilities\n

• Utilize your clinical skills and knowledge to independently assess patients, formulating psychosocial diagnoses based on comprehensive evaluations and ongoing engagement.

• Work collaboratively with patients to develop individualized, patient-centered treatment plans that reflect their unique needs, preferences, and goals.

• Implement evidence-based interventions and therapeutic techniques, including solution-focused therapy, motivational interviewing, and behavioral activation to foster positive mental health outcomes.

• Coordinate care with a multidisciplinary team which includes other clinicians, psychologists, and primary care providers, ensuring a collaborative approach to treatment that addresses all facets of patient health.

• Regularly assess treatment efficacy and patient progress, employing both virtual and in-person follow-ups to adapt treatment plans as necessary.

• Support medication management by providing education on psychotropic medications, focusing on adherence, managing side effects, and monitoring overall effectiveness in collaboration with prescribing medical providers.

Patient Population / Client Focus\n

The LCSW will primarily serve patients in the Hospice Center, a unique setting requiring sensitivity and understanding of end-of-life issues. Qualifications\n

Compensation and Benefits\n
