Key Responsibilities\n

• Utilize your education, training, and experience to independently formulate psychosocial diagnoses and create comprehensive treatment plans tailored to each patient’s needs.

• Engage collaboratively with patients to establish agreed-upon goals, employing evidence-based treatments such as motivational interviewing, solution-focused therapy, and behavioral activation.
\n
• Regularly track treatment progress, adjusting plans as necessary to ensure alignment with patient objectives, utilizing both virtual and in-person methods.
\n
• Coordinate care effectively with other health professionals, including psychologists, medical providers, and behavioral health peers at Walmart Health & Wellness, ensuring an integrated, holistic approach to patient care.

• Monitor medication adherence and discuss side effects with patients, facilitating effective communication with prescribing providers regarding treatment outcomes.
\n
• Continuously educate yourself on best practices in mental health care and maintain up-to-date knowledge of your field to provide informed support and intervention strategies.

Patient Population / Client Focus\n

In this role, you will primarily serve patients in a hospice setting, working closely with individuals facing serious health challenges. Your focus will be on providing compassionate support and mental health resources to enhance their quality of life and psychological resilience. This role requires sensitivity and understanding of the complex emotions experienced by patients and their families during critical decision-making and transitional periods.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

• Current, valid, and unrestricted licensure as a Licensed Clinical Social Worker (LCSW) to practice independently in Colorado.

• Minimum of one year of experience providing therapeutic services as an independently licensed clinician.

Compensation and Benefits\n
