About the Role

As an LPN at Haven Place, you will be responsible for providing direct patient care to our residents, including administering medications, wound care, and treatments. You will also be responsible for conducting vital signs, monitoring patient conditions, and reporting any changes to the healthcare team. Our night shift LPN position offers a unique opportunity to work closely with our healthcare team to ensure our residents receive the best possible care during the evening hours.

Responsibilities

• Provide direct patient care to residents, including administering medications, wound care, and treatments

• Conduct vital signs, monitor patient conditions, and report any changes to the healthcare team

• Perform nursing procedures as necessary, including checking patient vitals, taking temperatures, and administering oxygen therapy

• Collaborate with the healthcare team to develop and implement individualized care plans for each patient

• Maintain accurate and up-to-date records of patient care and treatment
